WHO declares international public health emergency as Ebola spreads across Central Africa and health agencies warn of wider regional risks

A rapidly expanding Ebola outbreak in Central Africa is drawing urgent international concern after the World Health Organization declared the situation a Public Health Emergency of International Concern (PHEIC), its highest global alert level. The outbreak, centred in the Democratic Republic of the Congo (DRC) and now linked to cases in Uganda, is unfolding amid growing fears that conflict, weak healthcare infrastructure, and reduced global health funding could complicate containment efforts and allow wider regional spread.

Health authorities confirmed that the current outbreak involves the rare Bundibugyo strain of Ebola virus disease, a variant for which no approved vaccine or targeted treatment currently exists. The declaration comes as suspected cases and deaths continue rising across eastern Congo, particularly in Ituri Province near the borders of Uganda and South Sudan. Multiple international health agencies are warning that the actual number of infections could be significantly higher than currently reported due to delayed detection and gaps in surveillance systems.

The outbreak was officially declared by Congolese authorities on May 15 after laboratory testing conducted by the National Institute of Biomedical Research confirmed Bundibugyo virus infections linked to clusters of severe illness and unexplained deaths. According to WHO and CDC advisories released this week, patients have presented with symptoms including fever, vomiting, bleeding, weakness, and rapid clinical deterioration.

As of May 21, WHO, Africa CDC, and multiple national health agencies reported hundreds of suspected cases and more than 100 deaths associated with the outbreak. Uganda has also confirmed imported cases connected to travel from Congo, including at least one fatality in Kampala. Reuters later reported a confirmed Ebola case appearing in South Kivu province, a significant geographic expansion far from the outbreak’s original epicentre.  

The growing crisis is unfolding in one of the most difficult outbreak environments in the world. Eastern Congo has faced years of armed conflict, population displacement, mining-related migration, and attacks on healthcare infrastructure. Public health officials say those conditions make contact tracing, isolation, and treatment far more difficult than during outbreaks in more stable regions.

The Bundibugyo strain also presents an additional scientific challenge because most recent Ebola preparedness systems and vaccine stockpiles were developed around the more common Zaire strain. While supportive care can improve survival rates, health agencies currently lack licensed vaccines specifically designed for the Bundibugyo variant.

The WHO warned that delayed detection may have allowed the virus to circulate undetected for weeks before the outbreak was formally recognized. Experts from Imperial College London noted that several initial patients reportedly died before formal testing identified Ebola as the cause, increasing the risk of silent transmission chains in nearby communities.  

The outbreak is already beginning to influence international travel and border policy decisions. The United States issued new health advisories and airport monitoring measures this week, while India convened emergency preparedness meetings involving state health authorities nationwide. Several countries are increasing airport screening protocols for travellers arriving from affected regions.  

Health agencies continue emphasizing that the overall global risk currently remains low, but officials are concerned about regional spread across East and Central Africa if containment measures fail to stabilize transmission quickly. Uganda’s previous experience handling Ebola outbreaks has helped health authorities rapidly activate emergency response systems, isolation centres, and surveillance operations.

The outbreak is also reigniting debate over the state of global public health preparedness following years of strained healthcare systems after COVID-19. Researchers and humanitarian organizations argue that reductions in international aid and disease surveillance funding may have weakened early outbreak detection capabilities in vulnerable regions. Some experts believe recent cuts to international health programs contributed to delayed identification of the current outbreak.  

At the same time, several separate disease threats are also drawing attention across Africa and other regions. Marburg virus outbreaks in Tanzania and Rwanda earlier this year forced emergency responses from regional governments and international agencies. Although those outbreaks were eventually declared over, health officials say the repeated appearance of hemorrhagic fever outbreaks demonstrates how vulnerable many regions remain to rapidly emerging infectious diseases.  

Scientists continue studying why outbreaks involving Ebola and related viruses appear repeatedly in parts of Central and East Africa. Researchers believe interactions between humans and wildlife reservoirs, combined with deforestation, mining activity, urbanization, and climate-related environmental changes, may increase the risk of zoonotic spillover events.

The current Ebola outbreak is also highlighting how quickly infectious disease emergencies can intersect with geopolitical tensions, migration policy, and economic stability. Airlines, border agencies, hospitals, and emergency planners across multiple continents are now reviewing response protocols as authorities attempt to avoid broader international spread while preventing unnecessary panic.

Public health officials continue stressing that Ebola does not spread through casual airborne transmission like COVID-19. Transmission typically occurs through direct contact with bodily fluids from infected individuals or contaminated materials. However, the virus remains highly lethal, with historical fatality rates for Bundibugyo outbreaks ranging between 30 and 50 percent depending on healthcare access and treatment speed.
